Your Recuperative Place to Heal Between Hospital and Home

Our transitional care program provides specialized rehabilitative care to individuals following a stay in a hospital. Working alongside you and your loved ones, our highly trained transitional care unit (TCU) team is experienced in creating a care plan which, from the very first day you arrive, helps prepare you for discharge back to your home.

Patients who come to the TCU must meet the admission criteria established. Once their condition has improved, they will either be discharged back to their homes or to more appropriate placement. They can also explore the option to stay on our campus long-term.

Transitional care is offered at the Lyngblomsten Care Center, where comfort and healing during your stay is our number-one goal. We are committed to providing the best care possible.

  • Rehab: physical, occupational, and speech therapies
  • Post-surgical care
  • Surgical drain management
  • Wound care (V.A.C. therapy)
  • Ostomy care
  • IV therapy: peripheral lines and central lines
  • Hemodialysis (off-site)
  • Non-invasive ventilation
  • Tracheostomy management
  • Tube feeding

Lyngblomsten Care Center TCU Room

Room Amenities

There are 22 private rooms and 4 semi-private rooms. Nine private rooms have zero-entry showers. The TCU also has a fully equipped kitchen and dining room used exclusively for transitional care. Patients are welcomed to participate in campus activities.

Each room comes with the following:

  • Adjustable bed (remote-control operated)
  • Overbed table with storage compartments and mirror
  • Reading light
  • Phone
  • Chair
  • Satellite TV
  • Laptops available for check out
  • WiFi

Patients are encouraged to bring personal items that will make the room as comfortable as possible during their stay. They also have access to all of Lyngblomsten’s campus amenities.

Cost & Payments

Daily rates are based on level of care. For short-term transitional care, we accept Medical Assistance, private insurance, HMOs, Medicare, and private/personal funds.

Admissions Criteria

  • Patient’s stay is covered under Medicare A/HMO
  • Patient’s condition allows for reasonable expectation of discharge. (Patients are allowed up to a 60-day stay)
  • Patient and/or patient’s support system demonstrates a willingness to participate actively in the plan of care
  • Patient does not demonstrate disruptive behavior (e.g., a recent baseline history of wandering) that would infringe on other patients’ stays

Discharge Criteria

Patients will be ready for discharge or transfer when they no longer meet any one of the admission criteria.
