Our transitional care program provides specialized rehabilitative care to individuals following a stay in a hospital. Working alongside you and your loved ones, our highly trained transitional care unit (TCU) team is experienced in creating a care plan which, from the very first day you arrive, helps prepare you for discharge back to your home.

Rehab: physical, occupational, and speech therapies
Post-surgical care
Surgical drain management
Wound care (V.A.C. therapy)
Ostomy care
IV therapy: peripheral lines and central lines
