Admission criteria for short-term care
- Patient's stay is covered under Medicare A/HMO.
- Patient’s condition allows for reasonable expectation of discharge. (Patients are allowed up to a 60-day stay.)
- Patient and/or patient’s support system demonstrates a willingness to participate actively in the plan of care.
- Patient does not demonstrate disruptive behavior (e.g., a recent baseline history of wandering) that would infringe on other patients’ stays.
Discharge
Patients will be ready for discharge or transfer when they no longer meet any one of the admission criteria.
Cost & Payments
Daily rates are based on level of care. For short-term transitional care, we accept Medical Assistance, private insurance, HMOs, Medicare, and private/personal funds.
