What is transitional care?
Transitional care is a recuperative place to heal between hospital and home after an injury or illness.
Since 2005, our transitional care program has provided specialized rehabilitative care to individuals following a stay in a hospital. Working alongside you and your loved ones, our highly trained transitional care unit (TCU) team is experienced in creating a care plan which, from the very first day you arrive, helps prepare you for discharge back to your home.
Patients who come to the TCU must meet the admission criteria established. Once their condition has improved, they will either be discharged back to their homes or to more appropriate placement. They will also have the option to stay on our campus long-term.
Comfort and healing during the stay with us is our number-one goal. We are committed to providing the best care possible, just as Lyngblomsten has done since 1906.

Please note: Lyngblomsten offers transitional care for older adults at its campus in St. Paul. This type of care is not available at its campus in Lino Lakes, Minnesota.
